About Your Insurance

Your insurance contract/card for medical bill coverage is a contractual relationship between you and your insurer/employer. All coordination of this relationship to obtain payment is your responsibility. We will help you with this work based on your relationship with us. This practice is out of network and has no contractual obligations with all insurances including Medicare and Medicaid.

1. Concierge Patients:

Full Work plan for prior authorizations: Unfortunately, we can offer no guarantee of success in authorization as this relationship and the legal enforcement of it is out of our control. We will however file all paperwork necessary to file for a prior authorization for services or medicines.
Administrative Forms and Paperwork: We are happy to fill out the forms you require as your concierge contract provides for this work.
Out of network Reimbursement: We are happy to fill out the forms you require as your concierge contract provides for this work. We can offer no guarantee of success in reimbursement as this relationship and the legal enforcement of it is out of our control. We will however file all paperwork necessary that you provide to us and will always release a HCFA forms as well.

2. Direct Primary Care:

We will however file all paperwork necessary to file for a prior authorization for services or medicines billed at an hourly rate of service with a minimum charge of 1 hour at the Medicare rate of 152 USD/hr .
Administrative Forms and Paperwork: We are happy to fill out the forms you require at a rate of 50 USD per form. You may also inquire about our concierge service where this service is included. Additional work would be payable at time of service at the hourly rate of 152 USD/Hr.
Out of network Reimbursement: We are happy to fill out the forms you require at a rate of 50 USD per encounter. We can offer no guarantee of success in reimbursement as this relationship and the legal enforcement of it is out of our control. We will however file all paperwork necessary that you provide to us and will always release a HCFA forms as well.
